Figure 5Periodontitis-initiated pathogenesis of RA. Oral microbiota-derived PPADs initiate modification of periodontal tissue proteins that can trigger inflammation in situ and evolve autoimmunity against citrullinated epitopes in synovial joints. Epigenetic signals and mechanisms support RA initiation and progression by generating such an epigenetic milieu that promotes expression of pro-inflammatory genes in immune cells and also fuels high proliferation of synovial fibroblasts.
