Importance: The American Urological Association guidelines recommend 24 or fewer hours of antimicrobial prophylaxis for most urologic procedures. Continuing antimicrobial therapy beyond 24 hours may carry more risks than advantages.Entities:

Periprocedural period is 1 to 2 hours before and/or during a qualifying urologic procedure.
Guideline-concordant periprocedural therapy was defined as the selection of an American Urological Association–recommended antimicrobial agent without known resistance to that agent or selection of a non–American Urological Association agent with an appropriate indication.
Guideline-concordant postprocedural therapy was defined as the discontinuation of an antimicrobial agent within 24 hours of the procedure’s completion or the documentation of an appropriate clinical indication or apparent need (based on medical record review) for continuing the antimicrobial agent for longer than 24 hours.

One site was selected from each quintile of total procedure volume, with 75 patients randomly selected for review from each site.
Periprocedural period is 1 to 2 hours before and/or during a qualifying urologic procedure.
Postprocedural period is any time after the procedure was completed.
