Abstract
Optimal performance of pediatric cardiothoracic computed tomography (CT) is technically challenging and may need different approaches for different types of CT scanners. To meet the technical demands and improve clinical standards, a practical, user-friendly, and vendor-specific guideline for pediatric cardiothoracic CT needs to be developed for children with congenital heart disease (CHD). In this article, we have attempted to describe such guideline based on the consensus of experts in the Asian Society of Cardiovascular Imaging CHD Study Group. This first part describes the imaging techniques of pediatric cardiothoracic CT, and it includes recommendations for patient preparation, scan techniques, radiation dose, intravenous injection protocol, post-processing, and vendor-specific protocols.Entities:

Vendor-Neutral Check Points for Optimal Pediatric Cardiothoracic CT
| 1. Patient preparation | Patient age, body size indices, developmental stage, cooperativity, vital signs |
| Patient isocentering in CT gantry | |
| ECG leads and arm positioning | |
| Patient immobilization | |
| (1) Sedation | Levels of sedation depending on scan techniques and institutional protocol |
| Minimal sedation is recommended in young children to avoid long-term neurodevelopmental impairment | |
| (2) Beta-blocker and nitroglycerin | For detailed evaluation of coronal artery in patients with high heart rates |
| Carefully consider benefits and risks | |
| Monitor blood pressure and heart rate | |
| 2. Scan techniques | Check specific clinical requests |
| Review patient history and results of previous studies | |
| (1) Non-ECG-synchronized spiral or helical scan | Extracardiac structures only |
| Cardiac structures exclusively in high-pitch dual-source scan | |
| (2) Prospectively ECG-triggered sequential scan | Intracardiac structures and coronary arteries |
| Functional evaluation | |
| Low radiation dose | |
| Possible stair-step and band artifacts | |
| (3) Retrospectively ECG-gated spiral or helical scan | Intracardiac structures and coronary arteries |
| Functional evaluation | |
| High radiation dose | |
| (4) Myocardial delayed enhancement scan | In select patients with suspected myocardial abnormalities |
| (5) Four-dimensional scan | Dynamic evaluation of airways, lungs, diaphragms, and cardiovascular structures |
| (6) Dual-energy scan | Pulmonary thromboembolism |
| Thrombosis in Fontan pathway | |
| 3. Radiation dose reduction and optimization | Follow ALARA principle |
| Use body size-adapted CT imaging protocols | |
| Use tube current modulation, including ECG-controlled version | |
| Use iterative reconstruction algorithm if available | |
| 4. Intravenous injection protocol | Tri-phasic injection protocol |
| Injection rates depending on angiocatheter size and CT indication | |
| Use special intravenous injection methods in patients with Fontan circulation |
ALARA = As Low As Reasonably Achievable, CT = computed tomography, ECG = electrocardiography
