Abstract
BACKGROUND: In Vietnam, we observed a high incidence of carbamazepine (CBZ)-induced severe cutaneous adverse drug reactions (SCARs)-Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN), and drug-induced hypersensitivity rash with eosinophilia and systemic symptoms (DRESS). In other Asian countries, HLA-B(*)1502 is an established risk factor for SCARs.Entities:

Clinical characteristics
SD, standard deviation; SJS, Stevens-Johnson syndrome; TEN, toxic epidermal necrolysis; DRESS, drug-induced hypersensitivity syndrome with rash, eosinophilia and systemic symptoms; BSA, body surface area; SCORTEN, severity-of-illness score for toxic epidermal necrolysis; NA, not applicable.
*Odds ratio, 9.78; 95% confidence interval, 1.17-81.43; p = 0.02.
Cross-reactivity
HLA-B genotypes in patients with SCARs
Serotype 1 corresponding to Allele 1 and Serotype 2 corresponding to Allele 2. 3 homozygous patients.
SCAR, severe cutaneous adverse drug reaction; SJS, Stevens-Johnson syndrome; TEN, toxic epidermal necrolysis; DRESS, drug-induced hypersensitivity syndrome with rash, eosinophilia and systemic symptoms.
*Four patients without HLA B*1502 (3 TEN, 1 DRESS).
Correlation between HLA-B*1502 and phenotypes
OR, odds ratio; CI, confidence interval; SJS, Stevens-Johnson syndrome; TEN, toxic epidermal necrolysis; DRESS, drug-induced hypersensitivity syndrome with rash, eosinophilia and systemic symptoms; SCAR, severe cutaneous adverse drug reaction.
*Sensitivity 91.4%, specificity 76.0%, positive predictive value 84.2%, and negative predictive value 86.4%. †Sensitivity 89.5%, specificity 76.0%, positive predictive value 85.0%, and negative predictive value 82.6%.
Frequencies of other HLA-B alleles in both groups
OR, odds ratio; CI, confidence interval.
