Abstract
PURPOSE: To develop a self-report version of the EQ-5D for younger respondents, named the EQ-5D-Y (Youth); to test its comprehensibility for children and adolescents and to compare results obtained using the standard adult EQ-5D and the EQ-5D-Y.Entities:

Definitions (condensed) of the concepts embodied in the EQ-5D and the EQ-5D-Y
| Content of the definitions for the original EQ-5D | Result of revision process—Transfer to EQ-5D-Y |
|---|---|
| Health/your own health state | |
| Suitable to adopt for EQ-5D-Y. The given understanding of ‘health’ and ‘your own health state’ emphasize the need of an appropriate self-report version and underlines that only health-related impairments are addressed |
| Mobility | |
| Suitable to adopt for EQ-5D-Y. However, minor modification necessary due to changes in wording. Definition of level 3 had to be modified since ‘I have a lot of problems walking about’ includes the ability to walk only under high effort (with or without aids) as well as being not able to walk at all (when confined to bed or a chair). In line with the adult version children, who are able to move around in a wheelchair are assigned to level 2 |
| Self-care | |
| Suitable to adopt for EQ-5D-Y. However, modifications were necessary due to changes in wording and in order to clarify the meaning in younger age groups |
| Usual activities | |
| Refers to the ability to participate in activities such as (house)work, study, leisure and social activities that are carried out regularly by the respondent personally | Suitable to adopt for EQ-5D-Y. However, examples were replaced by child-specific activities such as going to school, hobbies, sports, playing, doing things with family or friends |
| Pain & discomfort | |
| Suitable to adopt for EQ-5D-Y |
| Anxiety & depression | |
| Suitable to adopt for EQ-5D-Y, despite of the new wording Since the given definition of ‘anxiety’ corresponds the meaning of ‘feeling worried’ and since the described meaning of ‘depression’ complies with the sense of ‘sad or unhappy’ the descriptions have been retained unchanged with reference to the new wording |
| Some/moderate/extreme problems | |
| Definitions are suitable for the child-friendly version even though the wording in the levels were partly changed. ‘A lot of problems’ is comparable to ‘Extreme problems’ referring to a high level of impairment or the inability to perform the activities in question In general, the age-appropriate ability to perform certain activities has to be taken into account |
| Visual analogue scale (VAS) | |
| Definitions are suitable for the child-friendly version. However, again the age-appropriate ability to perform certain activities has to be taken into account regarding the term ‘independently’ |

| To help people say how good or bad a health state is, we have drawn a scale (rather like a thermometer) on which the best state you can imagine is marked 100 and the worst state you can imagine is marked 0 | We would like to know how good or bad your health is TODAY. This line is numbered from 0 to 100. 100 means the 0 means the Please mark an X on the line that shows how good or bad your health is TODAY. The best health you can imagine The worst health you can imagine |
We would like you to indicate on this scale how good or bad your own health is today, in your opinion. Please do this by drawing a line from the box below to whichever point on the scale indicates how good or bad your health state is today Best imaginable health state Worst imaginable health state |
