Abstract
OBJECTIVE: Implantation of the conceptus in a twin cesarean scar pregnancy (CSP) is considered the rarest type of ectopic pregnancy. Preserving the fetus in utero and effectively dealing with CSP can be challenging. This study aimed to determine the clinical value of ultrasonography by monitoring imaging changes in twin CSP following selective feticide.Entities:

Placental attachment scoring system.
| 0 | 1 | 2 | |
|---|---|---|---|
| Position of the placenta | Normal | Marginal placental previa or low lying placenta | Completely placenta previa |
| Thickness of the placenta | <3 cm | 3–5 cm | >5 cm |
| Continuity of the clear space | Continuous | Local interruption | Disappeared |
| Bladder line | Continuous | local interruption | Disappeared |
| Lacuna | None | Present | Fused with boiling water sign |
| Condition of the subplacental vascularity | Normal blood flow | Increased blood flow, forming a cluster | Emergence of “cross-border” blood vessels |
| Blood sinus of the cervix | None | Present | Fused with boiling water sign |
| Morphology of the cervix | Complete | Incomplete | Disappeared |
General information of the patients.
| Patient number | Age (years) | Number of deliveries | Number of CSs | Gestational age (weeks) | Blood loss (mL) | Type of placental attachment | Hysterectomy |

| 1 | 29 | 1 | 1 | 37+2 | 200 | Accreta | No |
| 2 | 36 | 1 | 1 | 29+2 | 3200 | Percreta | No |

CSs, cesarean sections.
Figure 1.Early pregnancy ultrasound image shows one fetus in cesarean scar pregnancy.
Figure 2.Ultrasound shows the fetal sac after feticide.
Figure 3.After feticide, color Doppler flow imaging shows local disappearance of the myometrium in a uterine scar.
Figure 4.After feticide, color Doppler flow imaging shows that peripheral blood flow is abundant.
