Abstract
PURPOSE: The purpose of this study was to use shear-wave elastography (SWE) to assess testicular stiffness changes during the Valsalva maneuver in adolescents and young adults with varicocele, to compare these changes according to clinical severity, and to evaluate the role of SWE in the diagnosis of varicocele.Entities:

P-values were calculated using a Spearman correlation test.
Volume asymmetry was calculated only in cases of preoperative unilateral varicocele (n=18).
Absolute change in Emean (kPa)=Emean during the Valsalva maneuver-Emean at rest.
Percentage change in Emean (%)=(Emean during the Valsalva maneuver-Emean at rest)/Emean at rest ×100.

The images show increased stiffness of the left testicle after the Valsalva maneuver compared with the stiffness at rest (absolute change in Emean, 0.9 kPa; percentage change in Emean, 34.6%), while no significant change was observed in the stiffness of the right testicle between the two conditions (absolute change in Emean, 0.03 kPa; percentage change in Emean, 1.2%). The images show increased stiffness of the left testicle after the Valsalva maneuver compared with the stiffness at rest (absolute change in Emean, 0.5 kPa; percentage change in Emean, 21.7%), while no significant change was observed in the stiffness of the right testicle between the two conditions (absolute change in Emean, 0.1 kPa; percentage change in Emean, 4.5%).
