Abstract
INTRODUCTION: Cardiocyte myofibrillolysis and interstitial fibrosis belong to histopathological changes in cardiomyopathies, leading to heart failure. AIM: To evaluate these changes in apical resection during left ventricular assist device (LVAD) implantation.Entities:

Fig. 1Digital image editing and analysis. Original image stained with Masson trichrome (at the top) is segmented into four tissue compartments (below): green stained collagen, blank space, normal, non-myofibrillolytic cardiomyocytes stained red-purple and myofibrillolytic cardiomyocytes stained rose. Detected color was covered by blue mask
Histopathological grading of dilated cardiomyopathy
| Grade/name | “First look” diagnosis | Cardiocyte sarcoplasm | Cardiocyte nucleus | Interstitium | Others |
|---|---|---|---|---|---|
| 0 No cardiomyopathy | Normal myocardium | Myofibrils poorly visible, dense sarcoplasm in almost all cardiocytes | Rather uniform oval or rounded. Chromatin uniform and delicately granular | Poorly visible, reduced to few thin collagen fibers between cardiocytes. Single fibroblasts | Single lymphocytes and/or other connective tissue cells inside interstitial network at the proximity of vessels |
| I Slight | Slight hypertrophy and patchy fibrosis. Practically uniform staining. Possible cardiomyopathy | Infrequent distension and attenuation of cardiocytes. Myofibrillolysis, premyocytolysis frequent. Unchanged cardiocytes not less than 30% | Some cardiocyte nuclei hyperchromatic, hypertrophied, enlarged | Fibrosis not exceeding nearly 10% of the tissue. Subtle collagen network and/or single scars. Non-numerous fibroblasts | Lymphocytes sporadically seen inside interstitial network, with no contact with cardiocytes. No active inflammatory foci |
| II Moderate | Image could suggest fibrosis. Tissue rarefied. Probable cardiomyopathy | Cardiocytes arranged loosely. Cardiocyte changes as above, but more intense. Unchanged cardiocytes below 30% | Cardiocyte nuclei enlarged, hyperchromatic, rounded or angular | Collagen as network or scars, forms groups of cardiocytes. Variable number of fibroblasts | As above. Pericytes and single mast cells could suggest inflammation |
| III Severe | Fibrosis with rare bright and enlarged cardiocytes. Tissue rarefied. Cardiomyopathy | Loose structure except scars. Myofibrillolysis and premyocytolysis as above. Unchanged cardiocytes rare | As above, also vesicular nuclei with marginated chromatin, and bizarre dense dark nuclei not rare | Dense and loose collagen surrounds cardiocytes. | As above. Visually increased number of capillaries. Focally subendocardial vessels |
Image analysis results from CM/VAD and control/comparative group, according to cardiomyopathy grades
| Grades | Cardiomyopathy grade 0 (no cardiomyopathy) | Cardiomyopathy grade 1 (mild cardiomyopathy) | Cardiomyopathy grade 2 (moderate cardiomyopathy) | Cardiomyopathy grade 3 (advanced cardiomyopathy) |
