Abstract
OBJECTIVES: Although rare, incidents of broken/dislodged fragment of various angioplasty hardwares, including catheters, guidewires, angioplasty balloons, and stents, are being increasingly reported in recent years. Since these broken fragments may cause life-threatening consequences for a patient, it is vital for an interventional cardiologist to be acquainted with different retrieval techniques. Here, we are reporting our observations of several incidents of device dislodgement/fracture during cardiac interventions and their retrieval using simple balloon method.Entities:

Fig. 1.(A) Significant occlusion in mid left anterior descending artery (LAD); (B) A 3 × 23 mm drug-eluting stent deployed in LAD lesion; (C) Detached balloon remained in left main coronary artery after post-dilatation; (D) Detached balloon was trapped between outer surface of the non-compliant balloon balloon and inner surface of the catheter; (E) Whole assembly was removed en masse through the radial sheath; (F) Retrieved balloon after its removal from the vascular system
Summary of individual cases of device retrieval using simple balloon method during cardiac procedures
| Case no. | Cardiac procedure | Name of dislodged/fractured cath-lab device | Patient’s demography | Size of balloon used for retrieval (mm) | Pressure used for balloon retrieval (atm) | Effectiveness endpoint of retrieval | Safety endpoint of retrieval |
|---|---|---|---|---|---|---|---|
| 1 | Coronary stent implantation | Detached predilatation PTCA balloon | 50/M | 2.5 × 10 | 14 | Achieved | Achieved |
| 2 | Coronary stent implantation | Detached postdilatation balloon | 62/F | 2.5 × 9 | 10 | Achieved | Achieved |
| 3 | Coronary stent implantation | Broken stent shaft | 40/M | 2.5 × 15 | 14 | Achieved | Achieved |
| 4 | Coronary stent implantation | Dislodged stents from stent balloon, occurring twice in same sitting | 46/F | 1.5 × 10 | 14 | Achieved | Achieved |
| 5 | Coronary stent implantation | Uncoiled guidewire | 70/M | 2.5 × 9 | 10 | Achieved | Achieved |
