Abstract
BACKGROUNDS: Some patients who undergo implantation of cardiac resynchronization therapy with defibrillator (CRT-D) survive long enough, thus requiring CRT-D battery replacement. Defibrillator therapy might become unnecessary in patients who have had significant clinical improvement and recovery of left ventricular ejection fraction (LVEF) after CRT-D implantation.Entities:

The data are presented as the number (percentage) or the mean ± standard deviation. CRT, cardiac resynchronization therapy; NYHA, New York Heart Association; LVEF, left ventricular ejection fraction; LVEDV, left ventricular end-diastolic volume; LVESV, left ventricular end-systolic volume; LBBB, left bundle branch block; BNP, brain natriuretic peptide; ACE-I, angiotensin-converting enzyme inhibitor; ARB, angiotensin-receptor blocker; MRA, mineralocorticoid receptor antagonist.
Fig. 1Kaplan-Meier curves showing ventricular tachycardia/ventricular fibrillation events (upper left) and hospitalization for heart failure (upper right) in patients who did and did not receive a downgrade. Chest X-ray in a representative case from initial implantation of CRT-D to a downgrade to CRT-P is shown (lower figure). VT/VF = ventricular tachycardia/ventricular fibrillation, HF = heart failure, CRT-D = cardiac resynchronization therapy with defibrillator, CRT-P = cardiac resynchronization therapy with pacemaker.
