Unadjusted Model includes Appalachian residence, violence (lifetime IPV, sexual assault, or childhood sexual abuse; yes vs. no) and interaction term for Appalachian residence and violence, Model 1 additionally adjusts for State registry (dichotomous: North Carolina versus Kentucky), time since cancer diagnosis as design elements, age (continuous: 18–79) and stage at diagnosis (I–IV), cancer site (see Table 1 for sites), race (dichotomous: non-Hispanic White versus other), and current smoking status (dichotomous: current smoker versus not current smoker)
Model 2 additionally adjusts for depression and stress at cancer diagnosis (continuous symptoms scores)
Model 3 additionally adjusts for highest education attainment (ordinal: < high school graduate, high school graduate, some college, college graduate, other including graduate or professional degree), monthly family income (ordinal: < $1000, $1000–$1999, $2000–$2999, $3000–$3999, $4000–$4999, $5000 or more, and private health insurance (yes vs. no)
