| Platelet count <100×109/L. | |
| Hemoglobin <8 g/dL | |
| History of allergy to heparin | |
| History of heparin induced thrombocytopenia | |
| Body weight <45 kg | |
| Uncontrolled severe hypertension (>160/110 mmHg) | |
| Creatinine clearance of <30 mL/min or serum creatinine >1.5× ULN | |
| Impaired liver function (transaminases >2× ULN) | |
| Bleeding risk (cerebral ischemia, intracerebral bleeding or gastrointestinal bleeding within the past 6 mon, neurosurgery or other surgery within the past 1 mon, an active peptic ulcer) | |
