Effects of yoga breathing exercises on pulmonary function in patients with Duchenne muscular dystrophy: an exploratory analysis.

OBJECTIVE: Duchenne muscular dystrophy (DMD) is the most common form of muscular dystrophy in children, and children with DMD die prematurely because of respiratory failure. We sought to determine the efficacy and safety of yoga breathing exercises, as well as the effects of those exercises on respiratory function, in such children.
METHODS: This was a prospective open-label study of patients with a confirmed diagnosis of DMD, recruited from among those followed at the neurology outpatient clinic of a university hospital in the city of São Paulo, Brazil. Participants were taught how to perform hatha yoga breathing exercises and were instructed to perform the exercises three times a day for 10 months.
RESULTS: Of the 76 patients who entered the study, 35 dropped out and 15 were unable to perform the breathing exercises, 26 having therefore completed the study (mean age, 9.5 ± 2.3 years; body mass index, 18.2 ± 3.8 kg/m(2)). The yoga breathing exercises resulted in a significant increase in FVC (% of predicted: 82.3 ± 18.6% at baseline vs. 90.3 ± 22.5% at 10 months later; p = 0.02) and FEV1 (% of predicted: 83.8 ± 16.6% at baseline vs. 90.1 ± 17.4% at 10 months later; p = 0.04).
CONCLUSIONS: Yoga breathing exercises can improve pulmonary function in patients with DMD.

Introduction

Duchenne muscular dystrophy (DMD) is the most common form of muscular dystrophy in children, occurring in 1 of every 3,000-3,500 male births.( , ) There is evidence that corticosteroid therapy has beneficial effects on the health and quality of life of patients with DMD and can decrease the requirement for nocturnal ventilation in such patients.( ) As a result of dystrophin deficiency, patients with DMD (including those who receive optimal treatment) experience a progressive loss of muscle fiber that eventually affects the respiratory muscles.( ) The resulting respiratory failure is the most common cause of premature death in patients with DMD.( , - )

Respiratory muscles are progressively compromised in children with DMD, the expiratory muscles being the most commonly affected.( ) Even short periods of physical inactivity can contribute to muscle weakness and reduced respiratory capacity.( - ) There is evidence that breathing exercises can improve respiratory function in patients with DMD.( , ) Hatha yoga is a broad philosophy that encompasses a series of breathing exercises aimed at improving the health of its practitioners. The objective of the present study was to determine whether a 10-month program of yoga breathing exercises that recruit inspiratory and expiratory muscles is safe for children with DMD and can improve their respiratory function.

Methods

The study sample consisted of consecutive patients treated at the neurology outpatient clinic of a university hospital in the city of São Paulo, Brazil. The inclusion criteria were as follows: having been diagnosed with DMD (as confirmed by molecular assessment of the skeletal muscle); being in the 6-14 year age bracket; and using corticosteroids regularly for at least 3 months. Severely ill children who were unable to perform the breathing exercises were excluded from the study. The study protocol was approved by the local research ethics committee, and the parents or legal guardians of all participants gave written informed consent.

The present study was conducted over a 10-month period, with clinical evaluations being performed in the morning at study entry (baseline) and at study termination. During the study period, the patients returned for clinical evaluations at regular intervals (of 1-2 months).

All children were individually taught how to perform the breathing exercises while sitting in a quiet room, practicing each exercise until they were able to perform it without supervision. The children were taught a new exercise at each clinical evaluation. The first breathing exercise that they were taught was kapalabhati, consisting of nasal exhalations produced by fast, vigorous contraction of the abdominal and pelvic muscles, followed by passive inhalations produced by relaxation of the recruited muscles. At 3 months after study entry, the children were taught another breathing exercise, which is known as uddiyana and consists of apnea after forced expiration, followed by thoracic expansion (achieved without inhalation) and voluntary glottic closure. At 6 months after study entry, the children were taught yet another breathing exercise, which is known as agnisara and consists of maximal contraction followed by abdominal projection during apnea after forced expiration. The participants were instructed to perform this sequence of exercises three times a day, every day, as follows: three series of 120 repetitions for kapalabhati; three 10-s repetitions for uddiyana; and three series of five movements for agnisara. Caregivers kept a diary, in which they marked an "x" every time the children performed the home exercises. The diaries were returned to the researchers on a monthly basis. We included only those children whose adherence to the exercise program was at least 75%.

Spirometry was performed with a dry bellows spirometer (Koko Spirometer; PDS Instrumentation, Inc., Louisville, CO, USA) in accordance with the American Thoracic Society/European Respiratory Society Task Force standards for lung function testing.( ) We measured FEV1 and FVC, predicted normal values being determined by the use of validated equations.( ) We measured MEP and MIP at the mouth using a portable spirometer (microQuark; Cosmed, Rome, Italy) under static conditions, in accordance with a validated method.( ) We measured MEP at TLC and MIP at functional residual capacity, the highest of three valid measurements being recorded. Before these measurements were obtained, patients were allowed to make at least three attempts. The results are expressed as relative values (percentages of the predicted values for age).

Normality was tested with the Kolmogorov-Smirnov test. The Student's t-test for repeated measures (baseline vs. 10 months after study entry) was used in order to evaluate the effects of the intervention on all physiological variables. The level of significance was set at p < 0.05. The results were analyzed with the Statistical Package for the Social Sciences, version 16.0 (SPSS Inc., Chicago, IL, USA).

Results

A total of 86 patients with a confirmed diagnosis of DMD were initially considered for inclusion. Of those, 10 were ineligible because they were not receiving corticosteroids. Therefore, 76 patients entered the study. Of those, 35 dropped out and 15 were unable to perform the exercises, being therefore excluded from the study. Many of the patients who dropped out lived in cities that are far from where the present study was conducted and were therefore unable to attend the scheduled clinical evaluations (Figure 1). The demographic characteristics and pulmonary function test results of the 76 patients who entered the study are presented in Table 1.

There were no significant differences between MEP at baseline and MEP at 10 months after study entry (63.9 ± 27.6% of predicted vs. 66.8 ± 27.6% of predicted; p > 0.05) or between MIP at baseline and MIP at 10 months after study entry (41.4 ± 13.7% of predicted vs. 43.7 ± 12.8% of predicted; p > 0.05). However, FVC at baseline was significantly lower than FVC at 10 months after study entry (82.3 ± 18.6% of predicted vs. 90.3 ± 22.5% of predicted; p = 0.02; Figure 2). Likewise, FEV1 at baseline was significantly lower than FEV1 at 10 months after study entry (83.8 ± 16.6% of predicted vs. 90.1 ± 17.4% of predicted; p = 0.04; Figure 3).

Discussion

The present study evaluated children with DMD receiving optimal medical therapy with corticosteroids. In children with DMD, respiratory function is expected to deteriorate over time, at a rate of 5% per year.( ) The novel findings of the present study include the fact that a large proportion of patients with DMD (82.6%) were able to learn how to perform yoga breathing exercises and the fact that pulmonary function can improve over a 10-month period, as evidenced by significant improvements in FEV1 and FVC.

Over the course of DMD, absolute values of lung capacity initially increase, then reach a plateau, and finally decrease. In contrast, relative values (proportional to the values predicted for age and gender) of all pulmonary function variables show a constant decline after the age of 5 years. ( , , ) For instance, respiratory capacity decreases by approximately 5% per year.( ) Because DMD is a relatively rare and progressive disease with an expected survival of 16-19 years,( ) the best evidence for standard of care and new treatments is primarily from observational studies. One group of authors( ) evaluated 40 DMD patients who were treated with corticosteroids in comparison with 34 who were not and concluded that lung function stabilization constitutes evidence of the beneficial effect of corticosteroids on DMD. Similar findings were recently reported for patients in Brazil. In our study, we included only DMD patients who were using corticosteroids regularly and used predicted values of pulmonary function as primary endpoints. In fact, it has been demonstrated that FVC (in % of predicted) decreases by approximately 4% per year in DMD patients.( ) In our patients, who participated in a 10-month yoga exercise program, lung function stabilized (which is, in and of itself, a positive effect) and there were increases in FEV1 and FVC (both in % of predicted). Therefore, although our study had no control group, the results should be interpreted as evidence that yoga breathing exercises have an additive positive effect on the lung function of DMD patients undergoing conventional treatment.

One study showed that respiratory muscle training can be beneficial to patients with DMD. ( ) Despite the lack of studies addressing the use of yoga breathing exercises as complementary therapy for DMD patients, there have been clinical respiratory studies of the topic, although the results have been controversial. In a recent review of the effects of breathing exercises on COPD,( ) such exercises were found to be effective in increasing the six-minute walk distance. In contrast, another review showed that yoga had no effect on cardiorespiratory variables in asthma patients.( ) However, those two reviews evaluated the effects of yoga in adults. In children with asthma, FEV1 was significantly increased after yoga training,( ) which included yoga postures and breathing exercises. To our knowledge, the present study is the first to address and confirm the safety and efficacy of yoga breathing exercises in children with DMD. The yoga breathing exercises used in the present study are unique and were chosen because children can learn them after only a few sessions and subsequently practice them by themselves without the need for additional equipment or continuous supervision, the exercise program being therefore accessible to a large number of patients. During the exercise known as kapalabhati, abdominal muscles are kept active, working rapidly and vigorously as expiratory muscles. Exhalation occurs below functional residual capacity and therefore becomes active, whereas inhalation becomes passive. This is in contrast with what occurs physiologically (inhalation being active and exhalation being passive). Kapalabhati therefore allows expiratory muscle training while the inspiratory muscles are at rest. Kapalabhati was the first exercise that the children were taught, being therefore the most practiced exercise and the exercise that probably contributed the most to the results obtained. Both uddiyana and agnisara have conditioning effects on the inspiratory muscles, the former focusing primarily on the intercostal respiratory muscles and the latter focusing primarily on the diaphragm.

Our study has strengths and limitations. The primary objective of the study was to show that yoga breathing exercises can be performed by patients with DMD. This hypothesis had to be carefully tested, particularly because DMD is a progressive, genetic disease, and breathing exercises could have deleterious effects on such patients (i.e., effects similar to those of overtraining in healthy individuals). In fact, the dropout rate was high in our sample. Although this might be due to difficulty breathing during the exercises, we believe that it was probably due to the fact that those patients were unable to attend the exercise sessions; that is, they had no means of transportation. In addition, only a small proportion of patients dropped out because they were unable to perform spirometry (either because they had respiratory problems or because they were unable to learn how to perform the exercises). Furthermore, there were no significant differences among the patients regarding any of the respiratory variables studied. Our study therefore showed that yoga breathing exercises are feasible, are harmless, and can actually improve respiratory function. Although our results clearly show that pulmonary function improved in terms of the predicted values-meaning that the increase was observed compared to each subject-randomized controlled studies are needed in order to confirm that. It is of note that DMD is a rare, fatal disease, and the best evidence for any form of treatment is from observational studies.

In conclusion, yoga exercises are feasible and can improve lung function in children with DMD. Further studies are needed in order to determine whether yoga exercises can improve quality of life and reduce the number of hospital admissions in such patients.

Introdução

A distrofia muscular de Duchenne (DMD) é a forma mais comum de distrofia muscular em crianças; de cada 3.000-3.500 recém-nascidos do sexo masculino, 1 é afetado.( , ) Há evidências de que a corticoterapia tem efeitos benéficos sobre a saúde e a qualidade de vida de pacientes com DMD e pode diminuir a necessidade de ventilação noturna nesses pacientes.( ) Como resultado da deficiência de distrofina, pacientes com DMD (inclusive aqueles que recebem o melhor tratamento) sofrem perda progressiva de fibras musculares, que acaba por afetar os músculos respiratórios.( ) A insuficiência respiratória resultante é a causa mais comum de morte prematura em pacientes com DMD.( , - )

Os músculos respiratórios são progressivamente afetados em crianças com DMD, sendo os músculos expiratórios os mais afetados.( ) Mesmo curtos períodos de inatividade física podem contribuir para que ocorra fraqueza muscular e redução da capacidade respiratória.( - ) Há evidências de que exercícios respiratórios podem melhorar a função respiratória em pacientes com DMD. ( , ) A hatha ioga é uma filosofia ampla que engloba uma série de exercícios respiratórios cujo objetivo é melhorar a saúde de seus praticantes. O objetivo do presente estudo foi determinar se um programa de 10 meses de exercícios respiratórios de ioga que recrutam a musculatura inspiratória e expiratória é seguro para crianças com DMD e pode melhorar sua função respiratória.

Métodos

A amostra foi composta por pacientes consecutivos tratados no ambulatório de neurologia de um hospital universitário na cidade de São Paulo (SP). Os critérios de inclusão foram os seguintes: diagnóstico de DMD (confirmado por avaliação molecular do músculo esquelético); faixa etária de 6-14 anos e uso regular de corticosteroides durante pelo menos 3 meses. Crianças gravemente doentes que não conseguiram realizar os exercícios respiratórios foram excluídas do estudo. O protocolo do estudo foi aprovado pelo comitê de ética em pesquisa da instituição, e todos os pais ou responsáveis assinaram um termo de consentimento livre e esclarecido.

O presente estudo foi realizado durante um período de 10 meses, com avaliações clínicas realizadas no período da manhã no início do estudo (antes do programa de exercícios) e no fim do estudo. Durante o estudo, os pacientes retornaram para avaliações clínicas a intervalos regulares (de 1-2 meses).

As crianças foram ensinadas a realizar os exercícios respiratórios individualmente, sentadas em uma sala silenciosa, praticando cada exercício até que fossem capazes de realizá-lo sem supervisão. As crianças aprenderam um exercício diferente a cada avaliação clínica. O primeiro exercício respiratório que lhes foi ensinado foi kapalabhati, que consiste em expirações nasais produzidas pela contração rápida e vigorosa dos músculos abdominais e pélvicos, seguidas de inspirações passivas produzidas pelo relaxamento dos músculos recrutados. Três meses após o início do estudo, as crianças foram ensinadas a realizar outro exercício respiratório, denominado uddiyana, que consiste em apneia após expiração forçada, seguida de expansão torácica (conseguida sem inspiração) e fechamento glótico voluntário. Seis meses após o início do estudo, as crianças foram ensinadas a realizar mais um exercício respiratório, denominado agnisara, que consiste em contração máxima seguida de projeção abdominal durante a apneia após expiração forçada. Os participantes foram instruídos a realizar essa sequência de exercícios três vezes por dia, todos os dias, da seguinte forma: três séries de 120 repetições de kapalabhati; três repetições de 10 s de uddiyana e três séries de cinco movimentos de agnisara. Em um diário, os cuidadores marcavam um "x" sempre que as crianças realizavam os exercícios de casa. Os diários eram devolvidos aos pesquisadores mensalmente. Foram incluídas apenas as crianças cuja adesão ao programa de exercícios foi de pelo menos 75%.

A espirometria foi realizada com um espirômetro seco de fole (Espirômetro Koko; PDS Instrumentation, Inc., Louisville, CO, EUA) conforme os padrões para testes de função pulmonar estabelecidos pela Força-Tarefa da American Thoracic Society/European Respiratory Society.( ) Foram medidos o VEF1 e a CVF, e os valores normais previstos foram determinados por meio de equações validadas.( ) A PEmáx e a PImáx foram medidas na boca com um espirômetro portátil (microQuark; Cosmed, Roma, Itália) em condições estáticas, de acordo com um método validado.( ) A PEmáx foi medida em CPT e a PImáx foi medida em capacidade residual funcional, sendo registrada a maior de três medidas válidas. Antes dessas medições, os pacientes podiam fazer pelo menos três tentativas. Os resultados são expressos na forma de valores relativos (porcentagens dos valores previstos para a idade).

A normalidade foi testada por meio do teste de Kolmogorov-Smirnov. O teste t de Student para medidas repetidas (antes do início do estudo vs. 10 meses depois) foi usado para avaliar os efeitos da intervenção sobre todas as variáveis fisiológicas. O nível de significância adotado foi de p < 0,05. Os resultados foram analisados por meio do programa Statistical Package for the Social Sciences, versão 16.0 (SPSS Inc., Chicago, IL, EUA).

Resultados

Considerou-se inicialmente a inclusão de 86 pacientes com diagnóstico confirmado de DMD. Desses 86 pacientes, 10 não estavam aptos a participar, pois não estavam recebendo tratamento com corticosteroides. Portanto, 76 pacientes entraram no estudo. Desses 76 pacientes, 35 abandonaram o estudo e 15 não conseguiram realizar os exercícios, sendo, portanto, excluídos do estudo. Muitos dos pacientes que abandonaram o estudo viviam em cidades distantes de onde o presente estudo foi realizado e, portanto, não conseguiram comparecer às avaliações clínicas agendadas (Figura 1). As características demográficas e os resultados dos testes de função pulmonar dos 76 pacientes que entraram no estudo são apresentados na Tabela 1.

Não houve diferenças significativas entre a PEmáx medida no início do estudo e aquela medida 10 meses depois (63,9 ± 27,6% do previsto vs. 66,8 ± 27,6% do previsto; p > 0,05) ou entre a PImáx medida no início do estudo e aquela medida 10 meses depois (41,4 ± 13,7% do previsto vs. 43,7 ± 12,8% do previsto; p > 0,05). No entanto, a CVF medida no início do estudo foi significativamente menor do que aquela medida 10 meses depois (82,3 ± 18,6% do previsto vs. 90,3 ± 22,5% do previsto; p = 0,02; Figura 2). Da mesma forma, o VEF1 medido no início do estudo foi significativamente menor do que aquele medido 10 meses depois (83,8 ± 16,6% do previsto vs. 90,1 ± 17,4% do previsto; p = 0,04; Figura 3).

Discussão

O presente estudo avaliou crianças com DMD que recebiam o melhor tratamento médico, com corticosteroides. Em crianças com DMD, espera-se que a função respiratória se deteriore com o tempo, a uma taxa de 5% por ano.( ) Dentre os achados inéditos do presente estudo estão o fato de que uma grande porcentagem de pacientes com DMD (82,6%) aprendeu a realizar exercícios respiratórios de ioga e o fato de que a função pulmonar pode melhorar ao longo de um período de 10 meses, conforme evidenciou a melhora significativa do VEF1 e da CVF.

Durante a DMD, os valores absolutos de capacidade pulmonar inicialmente aumentam; em seguida, estabilizam-se e, finalmente, diminuem. Por outro lado, os valores relativos (proporcionais aos valores previstos para a idade e o gênero) de todas as variáveis de função pulmonar diminuem constantemente após os 5 anos de idade.( , , ) Por exemplo, a capacidade respiratória diminui cerca de 5% por ano.( ) Como a DMD é uma doença relativamente rara e progressiva a qual se atribui uma expectativa de sobrevida de 16-19 anos,( ) as melhores evidências de padrão de tratamento e novos tratamentos provêm principalmente de estudos observacionais. Um grupo de autores( ) avaliou 40 pacientes com DMD tratados com corticosteroides em comparação com 34 que não o foram e concluiu que a estabilização da função pulmonar é evidência do efeito benéfico dos corticosteroides sobre a DMD. Achados semelhantes foram recentemente descritos em pacientes no Brasil. Em nosso estudo, incluímos apenas pacientes com DMD que usavam corticosteroides regularmente e usamos valores previstos de função pulmonar como desfechos primários. De fato, demonstrou-se que a CVF (em % do previsto) diminui cerca de 4% por ano em pacientes com DMD.( ) Em nossos pacientes, que participaram de um programa de 10 meses de exercícios de ioga, a função pulmonar se estabilizou (o que por si só é um efeito positivo) e tanto o VEF1 como a CVF (ambos em % do previsto) aumentaram. Portanto, embora nosso estudo não tenha tido um grupo controle, os resultados devem ser interpretados como evidência de que exercícios respiratórios de ioga têm efeito aditivo positivo sobre a função pulmonar de pacientes com DMD em tratamento convencional.

Um estudo mostrou que o treinamento muscular respiratório pode ser benéfico para pacientes com DMD.( ) Apesar da escassez de estudos sobre o uso de exercícios respiratórios de ioga como terapia complementar para pacientes com DMD, já houve estudos clínicos respiratórios sobre o assunto, embora os resultados tenham sido controversos. Em uma recente revisão dos efeitos de exercícios respiratórios sobre a DPOC,( ) constatou-se que tais exercícios são eficazes em aumentar a distância percorrida no teste de caminhada de seis minutos. Por outro lado, uma revisão mostrou que a ioga não teve efeito sobre variáveis cardiorrespiratórias em pacientes com asma.( ) No entanto, essas duas revisões avaliaram os efeitos da ioga em adultos. Em crianças com asma, o VEF1 aumentou significativamente após o treinamento de ioga,( ) que incluiu posturas de ioga e exercícios respiratórios. Até onde sabemos, o presente estudo é o primeiro a examinar e confirmar a segurança e a eficácia de exercícios respiratórios de ioga em crianças com DMD. Os exercícios respiratórios de ioga usados no presente estudo são únicos e foram escolhidos porque as crianças podem aprendê-los depois de apenas algumas sessões e, posteriormente, praticá-los sozinhas, sem a necessidade de equipamento adicional ou de supervisão contínua; o programa de exercícios é, portanto, acessível a um grande número de pacientes. Durante o exercício conhecido como kapalabhati, os músculos abdominais mantêm-se ativos, trabalhando rápida e vigorosamente como músculos expiratórios. A expiração ocorre abaixo da capacidade residual funcional e, portanto, torna-se ativa, ao passo que a inspiração se torna passiva. Isso está em contraste com o que ocorre fisiologicamente (isto é, inspiração ativa e expiração passiva). Portanto, kapalabhati permite o treinamento dos músculos expiratórios enquanto os músculos inspiratórios permanecem em repouso. Kapalabhati foi o primeiro exercício ensinado às crianças, sendo, portanto, o exercício mais praticado e o que provavelmente mais contribuiu para os resultados obtidos. Tanto uddiyana como agnisara condicionam a musculatura inspiratória, sendo este voltado principalmente ao diafragma e aquele aos músculos respiratórios intercostais.

Nosso estudo tem pontos fortes e limitações. O objetivo primário do estudo foi mostrar que exercícios respiratórios de ioga podem ser realizados por pacientes com DMD. Essa hipótese tinha de ser cuidadosamente testada, especialmente porque a DMD é uma doença genética, progressiva, e exercícios respiratórios poderiam ter efeitos deletérios nesses pacientes (isto é, efeitos semelhantes aos do supertreinamento em indivíduos saudáveis). De fato, a taxa de abandono foi alta em nossa amostra. Embora seja possível que isso tenha ocorrido em virtude de dificuldade para respirar durante os exercícios, acreditamos que tenha provavelmente ocorrido porque esses pacientes não puderam comparecer às sessões de exercício, isto é, não tinham meios para se transportar até o local. Além disso, apenas uma pequena porcentagem de pacientes abandonou o estudo porque não conseguiu realizar a espirometria (quer porque tinham problemas respiratórios, quer porque não aprenderam a realizar os exercícios). Portanto, nosso estudo mostrou que exercícios respiratórios de ioga são exequíveis, inofensivos e podem realmente melhorar a função respiratória. Embora nossos resultados mostrem claramente que a função pulmonar melhorou no que tange aos valores previstos, isto é, o aumento foi observado em relação a cada paciente, são necessários estudos controlados randomizados para confirmar isso. É digno de nota que a DMD é uma doença rara e fatal, e as melhores evidências de quaisquer formas de tratamento provêm de estudos observacionais.

Em suma, os exercícios de ioga são exequíveis e podem melhorar a função pulmonar em crianças com DMD. Mais estudos são necessários para determinar se exercícios de ioga podem melhorar a qualidade de vida e reduzir o número de internações hospitalares em tais pacientes.
