Abstract
BACKGROUND: Many patients with diabetes have poor blood pressure (BP) control. Pharmacological therapy is the cornerstone of effective BP treatment, yet there are high rates both of poor medication adherence and failure to intensify medications. Successful medication management requires an effective partnership between providers who initiate and increase doses of effective medications and patients who adhere to the regimen.Entities:

| 6 months preceding and following intervention period | Hierarchical logistic regression with a dichotomous dependent variable indicating whether each patient on a team is in control or not in each of the baseline and post intervention periods. The cluster identifier is the team, and the analysis will be stratified by site with fixed site effects. Using an analogous model, the primary hypothesis will be tested by examining the interaction between the treatment group variable and the time period variable at the laboratory measurement level. | |
