|---|---|---|---|
|
The CBI 19 Likert‐type questions | Individual burnout | 6 | How often do you feel worn out? |
| Professional burnout | 7 | Do you feel that every working hour is tiring for you? | |
| Customer‐related burnout | 6 | Are you tired of working with clients? | |
|
The WHOQOL‐BREF 26 Likert‐type questions | Bodily well‐being | 7 | To what extent do you feel that physical pain prevents you from doing what you need to do? |
| Mental well‐being | 6 | To what extent do you feel your life to be meaningful? | |
| Socialization | 3 | How satisfied are you with your personal relationships? | |

Nurse‐envisioned patient adverse events 4 Likert‐type questions | Medication errors | 1 | How often patients receive wrong medication or dose during last month? |
| Patient falls | 1 | How often patients experience falls after admission during last month? | |
| Hospital‐acquired infections | 1 | How often patients experience hospital‐acquired infection during last month? | |
| Pressure ulcers | 1 | How often patients experience pressure ulcer during last month? |
Two questions assess general quality of life and 24 questions assess four spheres: bodily well‐being, mental well‐being, socialization, and climate.
