Abstract
Background: Youth living with HIV in rural Rwanda experience poor clinical outcomes. In 2017, we implemented Adolescent Support Groups (ASGs), which provided economic incentives and peer support to youth aged 15-25.Entities:

| HIV + vulnerable youth aged 15 to 24. Vulnerability was defined as severe poverty, especially among those who were orphaned, disabled, or having an additional chronic disease. | Youth <15 and >25 were included, with vulnerability defined at nurses’ discretion | As in Kayonza |
| Each participating facility would create one group of 10 to 15 members. | Group size ranged from 4 to 33. | Group size ranged from 2 to 15. |
| |||
| Proportion of scheduled visits that were attended by group members in the last quarter. Typically, youth were scheduled with one visit per month. | Indicator scored as planned, but incentive amounts were sometimes doubled | As in Kayonza. |
| Proportion of money saved over a 6-month period compared to group target. Most groups would aim to save between 500 and 1000 RWF (approximately 0.50-1.00 USD) per member per month. | Indicator scored as planned but incentive amounts were sometimes doubled. | As in Kayonza. |
| Proportion of members with their last viral load suppressed (<20 copies/mL) at 12 months. | 15 000 RWF provided for each virally suppressed patient, with no stepwise decrease based on group performance. | Following a change in the national guidelines, viral suppression was defined as <200 copies/mL. |
| Number of groups initiated by the evaluated group Groups were eligible to receive 100 000 RWF per group. | Incentives were provided, but only one group was able to create a new group. | Indicator 4 discontinued |
Abbreviations: PLHIV, people living with HIV; RWF, Rwandan Francs.
Incentives were awarded in a stepwise manner. Groups scoring 100% received 100% of incentives, groups scoring 90%-<100% received 80% of incentives, groups scoring 80%-<90% received 60% of incentives, groups scoring 70%-<80% received 40% of incentives, groups scoring 60%-<70% received 20% of incentives, and groups scoring <60% received no incentives.
Under the National Differentiated Service Delivery Model, PLHIV with 2 consecutive suppressed viral loads who respect pharmacy appointments are eligible for multi-month ART prescriptions.
