Abstract
BACKGROUND: Monocyte Distribution Width (MDW), a simple proxy marker of innate monocyte activation, can be used for the early recognition of sepsis along with Procalcitonin. This study explored the added value of MDW as an early predictor of ensuing sepsis in patients hospitalised in an Intensive Care Unit.Entities:

Sepsis definition and considered laboratory parameters
| Definition | Reference | ||
|---|---|---|---|
| Sepsis | A life-threatening organ dysfunction caused by a dysregulated host response to infection | [ | |
| Organ Dysfunction | Organ dysfunction may be identified as an acute change in total SOFA score 2 points consequent to the infection | [ | |
| Septic Shock | Sepsis induced ipotension (Sepsis and vasopressor therapy needed to elevate MAP ≥65 mmHg and lactate > 2 mmol/L (18 mg/dL) despite adequate fluid resuscitation) or lactate above normal upper limits; urine output < 0.5 mL/kg/h for > 2 h despite adequate fluid resuscitation or creatinine > 2.0 mg/dL (176.8 μmol/L); acute lung injury, with PaO2/inspired oxygen fraction (FiO2) < 250 mmHg in the absence of pneumonia or acute lung injury with PaO2/FiO2 < 200 mmHg in the presence of pneumonia; bilirubin > 2.0 mg/dL (34.2 μmol/L); platelet counts < 100,000/mm3 | [ | |
| Microorganism associated with resistant infections | Gram negative | -Carbapenemase-producing -Extended-spectrum β-lactamase (ESBL)-producing -Acquired AmpC β-lactamase-producing -Carbapenemase-producing | [ |
| Gram positive | -Meticillin-resistant -Glycopeptide non-susceptible -Vancomycin resistant -Penicillin non-susceptible | [ | |
| Relevant considered laboratory parameters | MDW | Morphologic changes in the size of monocytes measured using the volume, conductivity, and scatter (VCS parameters) from the automated hematology analyzers. | [ |
| PCT | PCT > 0.5 ng/mL were considered the best cut off for sepsis management | [ |
