FIGURE 1A complicated case of a 7.5-year-old boy with multiple hereditary exostosis disease is presented. Due to ulnar shortening and forearm bowing (A), an external monofixator was applied for gradual ulna distraction (B). Despite a relatively slow lengthening speed of 0.5 mm/d, a poor regenerate formed, and nonunion eventually occurred (C). First, an attempt was made to enhance union by using an autologous iliac crest bone graft and plating (D). However, due to unknown reasons, the graft was resolved. Finally, bone consolidation was achieved by the use of a free vascularized fibula graft (E).
