Sepsis documentation: The clinician’s notes include a keyword ‘sepsis’ and two or more of the following criteria: temperature, heart rate, systolic blood pressure and respiratory rate.

SSI documentation: The clinician’s notes include any combination of the following keywords: ‘pus draining from the wound,’ ‘closed wound opened,’ ‘wound with a foul smell’ and ‘wound infection’ and one or more of the following symptoms: heat, redness, localized tenderness, purulent drainage, spontaneous dehiscence and operative findings indicative of infection.
Clinical progress documentation: patient history included, daily progress notes are written (every day patient was in the ward), doctors’ order documented (every day) and partogram utilized (among obstetric patients).
