Abstract
OBJECTIVES: To explore sequences of annual states of activity and sickness absence (SA) or disability pension (DP) (SA/DP) among working-aged people with multiple sclerosis (PwMS) as well as characteristics associated with the identified types of working-life sequences.Entities:

Observed activity state transition matrix* of the people with multiple sclerosis (n=2595) with five activity states†
| SA/DP | Mixed: SA/DP 180+ days | Mixed: SA/DP 30–180 days | Activity | Other | |
| SA/DP to | 0.90 | 0.09 | 0.02 | 0.00 | 0.00 |
| Mixed: SA/DP 180+ days to | 0.22 | 0.58 | 0.15 | 0.05 | 0.01 |
| Mixed: SA/DP 30–180 days to | 0.07 | 0.19 | 0.41 | 0.32 | 0.01 |
| Activity to | 0.00 | 0.03 | 0.11 | 0.85 | 0.01 |
| Other to | 0.02 | 0.06 | 0.06 | 0.23 | 0.63 |
*Each row sums to 1, with some rounding. This state transition matrix contains the observed transition rates pooled across the sequences; state transition probabilities may differ if looking at a specific position in the sequence due to the importance of timing within the sequence.
†SA/DP: ≥364 net days of sickness absence (SA)/disability pension (DP); Mixed: SA/DP 180+ days: predominately on SA/DP (>180 net days) but also some activity; Mixed: SA/DP 30–180 days: predominately in activity but with some SA/DP (30–180 net days); Activity: income from activity is defined as the combined sum of income from paid work, student allowances, unemployment compensation and parental leave, above 24% of the ‘price base amount’ (a set amount used for indexation in Sweden that changes with inflation each year); and Other: alive and living in Sweden but did not meet the definitions for the preceding states.

*Sociodemographic variables refer to 31 December/calendar year the year prior to multiple sclerosis (MS) diagnosis, and comorbidity characteristics were measured within the calendar year of MS diagnosis.
†The types of working-life sequences are ordered from most represented with being in activity (Stable High Activity) to being on full-time sickness absence (SA)/disability pension (DP) (Stable High SA/DP), with varying patterns of mixed activity and SA/DP in between.
‡Inclusion is the year of MS diagnosis.
§Income from activity is defined as the combined sum of income from paid work, student allowances, unemployment compensation and parental leave, above 24% of the ‘price base amount’ (a set amount used for indexation in Sweden that changes with inflation each year).
¶Anxiety/depression according to the Swedish Prescribed Drug Register (SPDR) by Anatomical Therapeutic Chemical Classification System (ATC) codes: N05BA01–N05BA56; N05BE01; N06AA01–N06AG02; N06AX01–N06AX11; N06AX13–N06AX26; and N06AX12.
**MS drugs according to the SPDR by ATC codes: L03AB07; L03AB08; L03AX13; L04AA23; and L04AA27.
††Pain drugs according to the SPDR by ATC codes: M01AB01–M01AX01; and N02AA01–N02AX99.
‡‡Comorbidity is according to a modified RxRisk Index constructed by ATC codes from the SPDR or whether in the Swedish Cancer Register, excluding MS drugs (ATC codes: L03AB07; L03AB08; L03AX13; L04AA23; and L04AA27). Comorbidity refers to the number of condition categories.
PPB, price base amount; SEK, Swedish krona.

*Adjusted for first multiple sclerosis (MS) year, age, and sex.
†The types of working-life sequences are ordered from most represented with being in activity (Stable High Activity) to being on full-time sickness absence (SA)/disability pension (DP) (Stable High SA/DP), with varying patterns of mixed activity and SA/DP in between, as well as the type Other.
‡As per the definition of being in no work and the states that Stable High SA/DP and Other largely comprise of, we were unable to meaningfully calculate statistics, especially with a reference group of Stable High Activity.
§Anxiety/depression according to the Swedish Prescribed Drug Register (SPDR) by Anatomical Therapeutic Chemical Classification System (ATC) codes: N05BA01–N05BA56; N05BE01; N06AA01–N06AG02; N06AX01–N06AX11; N06AX13–N06AX26; and N06AX12.
¶MS drugs according to the SPDR by ATC codes: L03AB07; L03AB08; L03AX13; L04AA23; and L04AA27.
**Pain drugs according to the SPDR by ATC codes: M01AB01–M01AX01; and N02AA01–N02AX99.
††Comorbidity is according to a modified RxRisk Index constructed by ATC codes from the SPDR or whether in the Swedish Cancer Register, excluding MS drugs (ATC codes L03AB07; L03AB08; L03AX13; L04AA23; and L04AA27). Comorbidity refers to the number of condition categories.
Mean differences and 95% CIs of annual incomes (thousand Swedish krona (SEK))* comparing the final study year (Y+5) with the first study year (Y−1), in total and by type of working-life sequence†

*Annual sums for the calendar year in 2015 SEK values inflated by Statistic Sweden’s Harmonised Index of Consumer Prices (€1 ≈ SEK9.3535). Extreme income values were trimmed at the 99% level for this study to reduce right skewness, with the maximum included annual values: income from activity SEK764 656; income from paid work SEK759 001; and disposable income (DI) SEK683 681.
†The types of working-life sequences are ordered from most represented with being in activity (Stable High Activity) to being on full-time sickness absence (SA)/disability pension (DP) (Stable High SA/DP), with varying patterns of mixed activity and SA/DP in between, as well as the type Other.
‡One individual with an imputed state in a year of comparison has been omitted from these analyses.
§Income from activity includes income within the year from paid work, student allowances, compensated parental leave and the active unemployment compensation.
¶Individualised measure of household DI, calculated as the after-tax sum of household incomes (incomes from paid work and transfer payments), adjusted for household size and the individual’s consumption weight. Statistics Sweden’s DI calculation was modified in 2004, allowing for negative values. Therefore, all negative values were set to 0 (<0.8% of entries), the previous lower limit.
diff, difference.
