aDisease burden was calculated by dividing the number of working years lost in terms of person-years by the total number of working years during the study period (also in terms of person-years) and results are expressed as fractions of 10,000 (ie, per myriad [proportion]). The observation periods were 42,234 person-years for 20–39 year old males, 226,094 person-years for 40–59 year old males, 28,062 person-years for 20–39 year old females, and 43,599 person-years for 40–59 year old females.
