INTRODUCTION: The Community Outreach and Patient Empowerment (COPE) intervention provides integrated outreach through community health representatives (CHRs) to people living with diabetes in Navajo Nation. Essential hypertension is the form of hypertension that has no identifiable cause.
Defined as ≥1 of the following diagnoses: acute myocardial infarction, coronary artery bypass surgery, coronary angioplasty, peripheral arterial disease, abdominal aortic aneurysm, carotid artery disease, cerebrovascular disease.
Evaluation at the closest available HbA1c measure before the enrollment date.
