BACKGROUNDS: Long-segment airway defect reconstruction, especially when carina is invaded, remains a challenge in clinical setting. Previous attempts at bioengineered carina reconstruction failed within 90 days due to delayed revascularization and recurrent infection.Entities:

Figure 1In-Vivo Bioreactor design for bioengineered long-segment airway reconstruction. (A) Custom-made nitinol stent wrapped in inner-layer ADM; (B) outer-layer ADM sutured onto the airway substitute together with two inserted Port-a-Cath catheters; (C) bioengineered airway, where trachea part is 6 cm long with a 1.8-cm inner diameter, right main bronchus is 1.5 cm long with a 1.4-cm inner diameter, and left main bronchus is 3.5 cm long with a 1.2-mm inner diameter; (D) bioengineered airway substitute repaired 8-cm trachea, carina, right main bronchus, and 3-cm left main bronchus defect. Continuous antibiotic perfusion between two ADM layers was created with two ambulatory pumps. ADM, acellularized dermis matrix.
Figure 2Treatment summary: postoperative VBS follow-up showed ADM biodegraded gradually and were replaced by well-vascularized tissue completely within 1 year. All three patients are back to normal life. ADM, acellularized dermis matrix.
