Importance: Evidence from national studies indicates systematic differences in hospitals in which racial/ethnic minorities receive care, with most care obtained in a small proportion of hospitals. Little is known about the source of these differences.Entities:

All estimates were obtained from 1 regression of the outcome measure (indicator of concordant destination ED or distance of EMS transport to ED) on race/ethnicity, age, sex, principal diagnosis, comorbidities, year of EMS transport, and zip code. All regressions applied sampling stratification and weights.

High-risk conditions were ED visits with the principal diagnosis of acute myocardial infarction, congestive heart failure, pneumonia, stroke, sepsis, and gastrointestinal bleeding.

Although the findings here were for walk-in ED visits, identification of reference ED in each zip code was based on emergency medical services–transported ED visits (as in Table 2 and Table 3), for comparability of concordance rates between emergency medical services–transported and walk-in ED visits.
Reported concordance rate and differences by race/ethnicity were adjusted for age, sex, principal diagnosis, comorbidities, Medicaid coverage, year of emergency medical services transport, and zip code location.
