Abstract
PURPOSE: Studies concerning tumor regression grade (TRG) after two-week course of radiotherapy (RT) are limited. We tried to assess associations of TRG and outcomes in patients with locally advanced rectal cancer (LARC) treated with preoperative two-week course of RT.Entities:

TRG tumor regression grade.
Figure 1Kaplan-Meier analysis of 356 rectal cancer patients treated with preoperative radiotherapy (30 Gy in 10 fractions) followed by surgery with curative intent according to tumor regression grade (TRG1-3/TRG4-5) showed statistically significant difference
(A) The 5-year OS of patients with TRG1-3 versus TRG4-5. (B) The 5-year DFS of patients with TRG1-3 versus TRG4-5. (C) The 5-year MFS of patients with TRG1-3 versus TRG4-5.
Figure 2Kaplan-Meier analysis of 356 rectal cancer patients treated with preoperative radiotherapy (30 Gy in 10 fractions) followed by surgery with curative intent according to pCR (pCR/non-pCR)
(A) The 5-year OS of patients with pCR versus non-pCR. (B) The 5-year DFS of patients with pCR versus non-pCR. (C) The 5-year MFS of patients with pCR versus non-pCR.

TRG tumor regression grade.
Figure 3Tumor regression grading of rectal tumors in patients treated preoperatively with radiotherapy
(TRG1) complete response with no residual cancer or fibrosis extending through the wall; (TRG2) presence of residual cancer cells scattered through the fibrosis; (TRG3) increased number of residual cancer cells, with fibrosis predominating; (TRG4) residual cancer outgrowing the fibrosis; (TRG5) absence of regressive changes.
