Abstract
BACKGROUND: Membranous nephropathy is a common cause of the nephrotic syndrome. Treatment with standard regimens fails to induce complete remission in most patients. We evaluated the efficacy of combination therapy with rituximab, low-dose, oral cyclophosphamide, and an accelerated prednisone taper (RCP) for the treatment of idiopathic membranous nephropathy.Entities:

Fig. 1Schematic representation of treatment regimen. Cyclophosphamide was administered orally at 2.5 mg/kg daily for 1 week, then 1.5 mg/kg for daily for 7 weeks. The cyclophosphamide dose was adjusted for renal function as delineated in the methods. Prednisone was initiated at 60 mg daily, tapered to 15 mg daily by 4 weeks, and then slowly tapered to complete a 6 month course as described.
