Abstract
BACKGROUND: Residual Kidney Function (RKF) is associated with survival benefits in haemodialysis (HD) but is difficult to measure without urine collection. Middle molecules such as Cystatin C and β2-microglobulin accumulate in renal disease and plasma levels have been used to estimate kidney function early in this condition. We investigated their use to estimate RKF in patients on HD.Entities:

GFRResidual = the measured residual GFR (mean of urea and creatinine clearances).r2 is the square of the Pearson correlation coefficient.
Fig 1Relationship of GFR with the reciprocal of pre-dialysis β2 microglobulin levels.
GFR shown is calculated from mean of urea and creatinine clearance. For the linear regression shown r2 was 0.67.
Fig 2Relationship of GFR with the reciprocal of pre-dialysis plasma Cystatin C levels.
GFR shown is calculated from the mean of urea and creatinine clearance. For the linear regression shown r2 was 0.5.
Β2-microglobulin level cut-off levels that may be used to identify patients with residual renal urea clearance.
For various levels of residual urea clearance, the cut-off β2-microglobulin level was determined that would yield a false positive rate (specificity) of 10%. For each level of urea clearance area under the curve (AUC) is shown for ROC analyses.
