Logistic regression model included age and gender. Household income was divided by quartiles and defined as low (Q1), middle (Q2 + Q3), and high (Q4), accordingly. Education levels were divided into low (≤ elementary school), middle (middle and high school), and high (≥ college). Middle income and middle education group served as the reference.
