Abstract
PURPOSE: To survey the standard keratoconus grading scale (Pentacam®-derived Amsler-Krumeich stages) compared to corneal irregularity indices and best spectacle-corrected distance visual acuity (CDVA). PATIENTS AND METHODS: Two-hundred and twelve keratoconus cases were evaluated for keratoconus grading, anterior surface irregularity indices (measured by Pentacam imaging), and subjective refraction (measured by CDVA). The correlations between CDVA, keratometry, and the Scheimpflug keratoconus grading and the seven anterior surface Pentacam-derived topometric indices - index of surface variance, index of vertical asymmetry, keratoconus index, central keratoconus index, index of height asymmetry, index of height decentration, and index of minimum radius of curvature - were analyzed using paired two-tailed t-tests, coefficient of determination (r(2)), and trendline linearity.Entities:

Classification stages of keratoconus adapted from the classical Amsler–Krumeich standards30
| CDVA | ISV | KI | Other indices | Rmin, mm | Retinoscopy signs | Cornea slit-lamp observations | |
|---|---|---|---|---|---|---|---|
| Pre-stage (early signs) | 20/20–20/15 | <30 | 1.04–1.07 | All four indices are “normal” | 7.8–6.7 | No clear light or shadow movement. Hint of “scissors” effect | Clear cornea, unobtrusive. Horizontal, oval, or round shades central or slightly decentered, when observed under direct ophthalmoscopy |
| Level 1 | 20/25–20/15 | 30–55 | 1.07–1.15 | Sometimes one value within the “abnormal” range | 7.5–6.5 | Distorted retinoscopic reflex. Scissors effect | Clear cornea. Fleischer’s ring at the apex base. Cone and cone base clearly visible with direct ophthalmoscopy. Decrease in apex thickness not visible, but measurable |
| Level 2 | 20/60–20/20 | 55–90 | 1.10–1.25 | Often one value within the “abnormal” range | 6.9–5.3 | Clear scissors effect, retinoscopy difficult to perform | Often cornea still clear, apex slightly thinner and eventually decentered. Partial or circular Fleischer’s ring. Vogt’s striae may be visible |
| Level 3 | 20/125–20/30 | 90–150 | 1.15–1.45 | At least one value within the “abnormal” range | 6.6–4.8 | Distinct scissors effect, retinoscopy nearly impossible to perform | Apex thinner, decentered, and often slightly cloudy. Clear and mostly circular Fleischer’s ring. Vogt’s striae clearly visible. Eventually Munson’s sign may appear |
| Level 4 | <20/400–20/100 | >150 | >1.50 | At least one value within the “abnormal” range | <5.00 | Retinoscopy impossible to perform | Cornea often scarred and opaque in the apex. Munson’s sign evident |
