Abstract
Blood cysts of the heart are rare benign tumors, usually involving the cardiac valves. They are found mainly in the first month of life and in children; and are rarely seen in adults. Here, we report a case of a blood cyst on the subvalvular apparatus of the mitral valve, which was incidentally discovered during chest computed tomography in a 47-year-old man. To the best of our knowledge, this is the first reported case of blood cyst of the heart in an adult in Korea.Entities:

Fig. 1Echocardiographic findings. A: A large cyst with small calcification (arrow) attached to the anterior mitral leaflet (transthoracic echocardiographic apical four-chamber view). B: A cyst (arrow) attached to the chordae tendinae and tip of papillary muscle of the anterior mitral leaflet (transesophageal echocardiographic five-chamber view). LA: left atrium, LV: left ventricle.
Fig. 2Cardiac magnetic resonance imaging. A: 18 × 15 mm sized, lobulated cystic mass (arrow) in the left ventricle. The mass was isointense on T1 weighted sequence. B: The mass (arrow) was hyperintense on T2 weighted sequence. C and D: The mass (arrow) did not show contrast enhancement. LA: left atrium, LV: left ventricle, RA: right atrium, RV: right ventricle.
Fig. 3Surgical and histological findings. A: A bluish cystic mass was seen attached to the chordal structure of mitral valve and tip of the papillary muscle. B and C: Cystic mass was excised and revealed old blood with a yellowish calcified body in the cyst. D: Cystic wall composed of dense connective tissue with flat lining cells. E: Flat lining cells were stained with antibody CD31. CD31: cluster of differentiation 31.
