Adjusted for age (in continuous variable), body mass index (<21, 21–22.9, 23–24.9, 25–26.9, 27.0–29.9, 30.0–32.9, 33–34.9, ≥35 kg/m2), smoking (never, past, current with 1–14, 15–24, ≥25 cigs/day), alcohol intake (no, <4.9, 5.0–9.9, or ≥10.0 g/d), and physical activity (<3, 3.0–8.9, 9.0–17.9, 18.0–26.9 or ≥27.0 metabolic equivalent hours/wk). The covariates were time varying and updated in the analysis.
