August 1995 through November 1999, covering 17 counties: Alameda, Contra Costa, Marin, Merced, Monterey, Napa, Sacramento, San Benito, San Francisco, San Joaquin, San Mateo, Santa Clara, Santa Cruz, Solano, Sonoma, Stanislaus, and Yolo.
December 1999 through December 2005, covering 35 counties: 17 counties from phase 1, plus Amador, Butte, Calaveras, Colusa, El Dorado, Fresno, Glenn, Kern, Kings, Madera, Mariposa, Nevada, Placer, San Luis Obispo, Sutter, Tulare, Tuolumne, and Yuba.
