Putting patients and families at the center of care.

Abstract

The authors describe how a patient- and family-centered model of care was introduced and adopted by their healthcare system. Since the model was introduced 2 years ago, it has been embraced by leaders and staff on both hospital campuses; patients and family members routinely consult with hospital committees, departments, and improvement task forces; and numerous clinical programs and units have modified their policies and practices to make them more patient and family centered.
