Abstract
An elderly male with a mass in left gluteal region with pain in this zone to radiate down the posterior aspect of his left thigh that is aggravated by dorsiflexion and extension. Thinking in diagnosis of soft tissue tumor the surgical approach was transgluteal rute finding a sciatic hernia (suprapiriformis type): the sac was contain small bowell. Closure of the defect by ligation of the sac and approximate piriformis and minor gluteus muscles. The postoperative recovery was uneventful and the follow-up during one year no evidence for recurrent hernia is noted. We inform this case because it's rarity and present in a male patient without predisponent factors. The etiology, diagnosis and treatment is discussed.Entities:
