Abstract
Anterior commissurotomy for the relief of intractable lower half and midline pain has been found to be a very satisfactory procedure in a small group of severely incapacitated patients. The presumed anatomic rationale for undertaking the procedure has not been confirmed by postoperative sensory examinations. Alternative explanations for its effectiveness remain conjectural.
