The total PHQ−4 score complements the subscale scores as an overall measure of symptom burden, functional impairment and disability. Total score is calculated by summing together the scores of each of the 4 items (1. Feeling nervous, anxious or on edge; 2. Not being able to stop or control worrying, 3. Not being able to stop or control worrying, 4. Little interest or pleasure in doing things) in PHQ-4. Scores are rated as normal (0–2), mild (3–5), moderate (6–8), and severe (9–12). Total score ≥3 for first 2 items suggests anxiety. Total score ≥3 for last 2 items suggests depression.
