Abstract
Small cell osteosarcoma is a rare sub-type of osteogenic malignant tumors. Rib localization is uncommon. Histological examination is an important step to make the difference with similar tumors like Ewing's sarcoma. Surgical resection is the unique curative treatment. The prognosis remains poor even with adjuvant treatment.Entities:

FIGURE 1(A) Biopsy specimen showing a highly cellular tumor infiltrating the skeletal muscle (asterisk) (H&Ex50); (B) the tumor is composed of uniform small round cells with hyperchromatic nuclei and indistinct cytoplasmic borders (H&Ex200); (C) note the presence of mitosis (arrows) and the absence of osteoid production (H&Ex400); (D) tumor cells show a diffuse membranous staining for CD99 (x400)

FIGURE 3Per operative views: (A): absence of liver invasion. (B): coverage by a polypropylene mesh and latissimus dorsi muscle flap
FIGURE 4Surgically resected specimen after neoadjuvant treatment: (A) at gross examination the sectioned specimen shows a large tan and hard tumor infiltrating the soft tissue (arrow) with few areas of yellowish necrosis (arrowhead); (B) microscopic examination shows a viable proliferation of uniform small round cells with rare foci of necrosis (asterisk) (H&Ex50); (C) closely packed tumor cells are interspaced with foci of osteoid production (arrows) (H&Ex100); (D) note the lace‐like osteoid deposited in between the uniform round cells (H&Ex400)
