Abstract
Background: Radiation after resection of an atypical lipomatous tumor (ALT) is controversial. This study evaluates local control and complications after the first resection of ALTs of the extremity with or without adjuvant radiation.Entities:

The 5-yr LRFS (95% CI) was 98% (88–99) in the XRT group and 92% (77–97) in the no-XRT group (p=0.21). The 10-yr LRFS was 98% (88–99) in the XRT group and 41% (17–64) in the no-XRT group (p < 0.001). Figure 3Complication severity by radiation dose. The box plot shows the median radiation dose received for patients based upon their complication severity (no complication, medical management required, and surgical management required). Eighty-seven of 102 patients had radiation dose data. Each dot represents a single patient's radiation dose. The vertical line within each box represents the median dose for that severity group, and the left and right ends of the box represent the interquartile range (IQR). The median radiation dose received for all patients was 60 Gy (55–66). The median doses for the none, medical management, and surgical management complication groups were 60 Gy (50.4–64.9), 64 Gy (60–66), and 64.1 (61–66.3), respectively. No differences in dose were found between the three groups (p=0.11). Spearman's rank correlation between radiation dose and complication severity was ρ=0.24(p=0.02), indicating a modest positive correlation.
