Abstract
Objective: This study aimed to report on 15 Japanese patients with acrodysostosis and pseudohypoparathyroidism (PHP) and analyze them using the newly proposed classification of the EuroPHP network to determine whether this classification system is suitable for Japanese patients. Design: We divided the patients into three groups based on hormone resistance, the number of fingers with short metacarpals, the existence of cone-shaped epiphyses and gene defects.Entities:

Figure 1Hand X-rays of patients A-1 in childhood and adulthood and B-1 in adulthood. Unlike in B-1, there is severe shortening of metacarpal and phalangeal bones and cone-shaped epiphyses in case A-1, in which a PDE4D pathogenic variant was detected.
Figure 2Photographs of face and X-rays of the hands of patients in group B. Shortening of metacarpal bones and cone-shaped epiphyses in fingers II–V is observed in all patients except B-3, with such shortening only in fingers IV–V, in whom a PRKAR1A pathogenic variant was detected.
Figure 3Facial photographs and X-rays of the hands of patients in group C. Shortening of metacarpal bones in fingers II–V can be seen in all cases except C-1 and his mother. Shortening of metacarpal bones in fingers III–V and cone-shaped epiphyses can be observed in C-1, in whom a GNAS pathogenic variant was detected. The serum Ca level was in the normal range and no CNS calcification was observed in patient C-1, his mother or in C-2.
