Figure 1The decision process regarding optimal timing of left ventricular assist device (LVAD) support before transplantation weighs the benefits of functional, organ, and psychologic recovery from advanced heart failure versus the risks associated with the well-known adverse event profile of durable LVADs. Within this paradigm there are several other important elements in the formula, including the possibility of myocardial recovery, center-dependent factors such as waitlist management and allocation strategies, and patient-dependent factors such as sensitization and psychosocial issues.
