Abstract
A subaortic left ventricular diverticulum (SLVD) represents an extremely rare congenital anomaly. It can be asymptomatic but sometimes develops fatal complications. Treatment has been debated due to limited experience. We present the successful treatment of a giant SLVD with aortic regurgitation and stenosis and ascending aorta dilatation. Our goal is to improve understanding of this rare entity.Entities:

Figure 1:Preoperative imaging of a subaortic left ventricular diverticulum. (A, B) Computed tomography scan showing a subaortic left ventricular diverticulum; (C) echocardiogram showing a subaortic left ventricular diverticulum with to and fro shunt through the orifice. LA: left atrium; LV: left ventricle; LVOT: left ventricular outflow tract; RV: right ventricle; SLVD: subaortic left ventricular diverticulum.
Figure 2:Intraoperative and postoperative findings of a subaortic left ventricular diverticulum. (A) orifice under noncoronary cusp; (B, C) postoperative CT scan and echocardiogram showing diverticulum resected with normally functioning artificial aortic valve. AO: aorta; LA: left atrium; LV: left ventricle; RA: right atrium; RV: right ventricle.
