Abstract
Context: Recanalization failure rate in mechanical thrombectomy (MT) for large vessel occlusions is up to 30%. Outcome greatly depends on recanalization success and, thus, there is an urgent need to adopt new strategies to improve recanalization. Aims: To report on the feasibility, safety, and outcome of rescue strategies (stenting and/or angioplasty) in cases of failed MT for acute ischemic stroke (AIS) in anterior circulation. Figure 1(a) and (b) Diffusion-weighted images show the right MCA watershed, caudate nucleus, and putaminal infarcts. (c) and (d) Time-of-flight magnetic resonance angiogram demonstrates right middle cerebral artery (MCA) occlusion. (e) RAPID processing of perfusion and diffusion images showing a large area of diffusion/perfusion mismatch corresponding to the volume of 184 ml
Figure 2(a) Preprocedure digital subtraction angiogram showing the right MCA occlusion. (b) First pass of mechanical thrombectomy done with contact aspiration. (c) Angiogram after first pass. (d) Second pass using stentriever. (e) Angiogram after second pass showing severe stenosis of middle segment of MCA with good antegrade flow. (f) Subsequent angiogram showing reocclusion of the MCA
Figure 3(a) Balloon angioplasty of MCA. (b) After placement of rescue stent. (c) Angiogram after rescue stenting shows mTICI 3 flow. (d) and (e) Computed tomography of brain done after 24 h does not show increase in the size of infarction of hemorrhage
