Abstract
OBJECTIVE: Knee osteochondritis dissecans (OCD) is a still poorly understood pathological condition of the articular subchondral bone and its overlying cartilage. Patellofemoral involvement accounts for less than 1% of cases; tibial plateau and multifocal involvement is an even rarer instance. The purpose of this study is to review what is currently known about patellofemoral OCD (PF-OCD) and to present an unusual case of PF-OCD which progressed to become multifocal in an adult female patient.Entities:

Outcome of Surgical Treatment of Patellofemoral OCD.
| Year | Authors | Procedure | N° of Patients | Staging of Lesion | Outcome | Follow-Up | Conclusions |
|---|---|---|---|---|---|---|---|
| 1988 | Schwarz | Curettage | 25 | Unreported | 38% good or excellent result | 73 months | Persistent pain with restricted function was common |
| 1990 | Smith | Drilling | 13 | Unreported | Drilling failed in 2 of the 3 patients | 5 years | Results of treatment have been imperfect. |
| 1994 | Arandes | Excision | 7 | Unreported | Pain resolution, no activity restrictions | 4 | - |
| 2000 | Peters | Chondroplasty, loose bodies removal, drilling, lateral retinacular release | 37 | Craters with or without a separate fragment | Treatment generally improved the symptoms, but patients with articular cartilage loss had persistent patellofemoral crepitus and discomfort. | Unreported | Operative intervention is indicated for patients with mechanical symptoms or failure of nonoperative treatment |
| 2010 | Visonà | Mosaicoplasty | 6 | ICRS grade IV | IKDC from 37.2 to 66.3 | 26 months | Satisfying results |
| 2014 | Wall | Drilling | 12 | Unreported | 67% healed with return to full activities | 3 years | Standardization of treatment approaches is still needed |
| 2015 | Kramer | 76%: transarticular drilling ± fixation | 26 | Average size 12.3 x 14.4 mm | 48% pain free | 3.8 years | 85% returned to sports |
| 2018 | Price | Fixation with bioabsorbable nails + drilling, microfracture, debridement, chondroplasty, and removal of loose bodies | 30 | Unreported | All patients returned to sports | 21 months | All patients showed radiologic and/or clinical indications of healing |
