Different Types of Skin Lesions in Multicentric Castleman Disease.

A 60-year-old man with multicentric Castleman disease (MCD) presented to our department for a routine visit and complained of thigh pain. A year ago, he had been diagnosed with MCD owing to systemic lymphadenopathy, resulting from a lymph node biopsy compatible with a plasma cell type of MCD and elevated serum levels of CRP, IgG, and IL-6. In addition, generalized lichenification (Picture 1) was detected, and findings of a skin biopsy were compatible with superficial perivascular dermatitis. After steroid treatment, the medical problem was completely resolved. On admission, he was administered prednisone 5 mg/day. His right thigh was warm and tender, with signs of post-inflammatory hyperpigmentation (Picture 2). Right-dominant inguinal lymphadenopathy was palpated on both sides. Serum CRP levels were elevated to 8.1 mg/dL. Short Tau Inversion Recovery (STIR) Magnetic Resonance Imaging (MRI) of the right thigh revealed skin inflammation around it (Picture 3, 4). We suspected skin lesions associated with MCD and increased the dose of prednisone to 15 mg/day. After one week, the right thigh inflammation was resolved, as confirmed by a physical examination and MRI. There are several types of MCD-associated skin lesions (1,2), and we encountered different ones in the same patient.
