Abstract
INTRODUCTION: Recent medical literature has drawn attention to the possible influence of COVID-19 on the course of pregnancies. As the coherence of results seems to vary, especially in relation to first and second trimester pregnancies, a concise qualitative systematic review can shed light on the most recent data.Entities:

CERQual assessment of review findings
| Review findings | CERQual assessment of confidence in the evidence | Explanation of CERQual assessment | Studies contributing to the review finding |
|---|---|---|---|
| Symptoms: | |||
| Majority of pregnant women who contract COVID-19 during the 1st or 2nd trimester are symptomatic with minor symptoms on admission. | Moderate | 7 studies with minor to moderate methodological limitations, of which selection bias was noted due to the studies pertaining to hospitalized patients only. Patient exposure history prior to onset of symptoms was largely under-reported. Relatively adequate data pertaining to 177 1st/2nd trimester pregnant COVID-19 patients. High coherence | 17, 22–44 |
| Least common: sore throat, loss of taste or smell, nausea, asthenia, tachycardia | |||
| Pregnancy-related complications: | |||
| A wide range of multisystem complications such as preeclampsia, DIC, hyperemesis gravidarum, hypothyroidism, gestational diabetes, neonatal jaundice are noted | Low | 5 studies with minor to moderate methodological limitations. 4 studies were admitted with severe cases of COVID-19, raising concern for potential selection bias. Selective reporting bias due to gaps and inconsistency in reporting of pregnancy-related complications. Short follow-up of patients (< 1 month) and incomplete/ongoing course of pregnancy (as of date of publication) largely limit the reflectiveness of findings. Inadequate data pertaining to only 19 pregnant COVID-19 patients. Low to moderate coherence | 22–27, 36, 37, 39–41 |
| Outcome: | |||
| Maternal outcome: The short term survival rate amongst mothers is estimated to be very high with complete discharge from the hospital | Low | 11 studies reported no further follow-up in regards to the mother or neonates’ wellbeing after discharge. Such limitations make findings incomplete for the whole course of pregnancy. 10 studies admitted severe cases, hence concerns for selection bias. Neonatal status was clarified in only 60 out of the 223 neonates, raising concerns for inadequacy of data. Moderate to high coherence in maternal outcome and moderate coherence in neonatal outcome | 17, 22–33, 36, 37, 39–42 |
| Neonatal outcome: A risk for miscarriage or stillbirth is present. Majority of pregnancies remain uneventful for neonatal health upon mother hospitalization. Successful delivery of healthy neonates has been recorded | |||
| Laboratory and radiological findings: | |||
| Normal laboratory or radiological findings may occur. | Low | 7 studies with minor to moderate methodological limitations. Selection bias due to admission of strictly severe cases was the most prominent methodological concern. Majority of data stemming from two geographic regions (China and USA), raising concern for lack of geographic diversity. Inadequate data pertaining to only 28 1st/2nd trimester COVID-19 patients. Moderate coherence | 22, 23, 25, 26, 28–30, 32–34, 36, 37, 40 |
