A symptomatic case related to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was defined as the presence of coronavirus disease 2019 (Covid-19) symptoms (observed by prison health care staff) or objective signs of fever (a temperature of ≥37.8°C) or hypoxemia (an oxygen saturation of <94%) within 3 days before to 10 days after the infection was initially confirmed.
Hospitalization related to SARS-CoV-2 infection was defined as a hospitalization with Covid-19 symptoms that occurred within 3 days before to 14 days after the infection was initially confirmed.
Effectiveness estimates were calculated with the use of a propensity-score–matching approach in which unvaccinated control residents were matched, with replacement, to vaccinated residents in a 1:1 ratio, on the basis of variables associated with the probability of vaccination, specifically, previous confirmed infection, age, race or ethnic group (Hispanic, non-Hispanic Black, non-Hispanic White, or non-Hispanic other), an index of coexisting conditions developed by the California Department of Corrections and Rehabilitation and used in prioritizing vaccinations (details are provided in Table S1 in the Supplementary Appendix), room type (cell or dormitory), security level (1, 2, 3, or 4), and undertesting during the winter outbreak (lower quartile for testing frequency). Estimates were adjusted for prison facility of residence, and outcomes were evaluated within the total cohort regardless of testing status. (Results of a sensitivity analysis restricted to residents who underwent testing are available in the Supplementary Appendix.) We estimated vaccine effectiveness for each outcome as 1 minus the risk ratio, expressed as a percentage; 95% confidence intervals were calculated with the use of cluster-robust standard errors.
