BACKGROUND: Due to the rising number of acutely hospitalised older people in the coming years, there is increased interest in tailoring care to the individual goals and preferences of patients in order to reach patient-centred care. Percentage is calculated based on the number of patients who reported that a certain goal played a role to them during the baseline interview as a percentage of the total number of follow up. The following columns show the contribution of the hospitalisation to achieving the goals. The goals that applied to at least 25% of the patients during the baseline interview are shown.
