Fig. 2A 4-mm punch biopsy demonstrated orthokeratotic hyperkeratosis, acanthosis, hypergranulosis, and vertically oriented collagen bundles in the papillary dermis, consistent with lichen simplex chronicus (hematoxylin-eosin, 100×).
