Rectal foreign bodies are commonly observed in middle‐aged men in the context of autoerotic instrumentation. Previous studies have reported a variety of objects in patients. , , We present the case of a 53‐year‐old man who lost a long sex toy in his rectum; it was inserted in his anus by his male partner for sexual stimulation and could not be removed. He visited our hospital 24 h later. On initial examination, he complained of severe lower abdominal pain. The foreign body was not palpable in a rectal examination and could not be located by anoscopy. Computed tomography revealed a sex toy in the shape of male genitalia on both sides (44 × 4 cm), located below the liver from the pelvis (Fig. 1A, 3‐D reconstruction; Fig. 1B, sagittal view). Free air was noted (Fig. 1B, yellow arrow). Endoscopy carried out under general anesthesia did not reveal the foreign body. Therefore, we undertook a laparotomy, and the foreign body was located in the abdominal cavity and removed (Fig. 1C). A 5‐cm perforation of the rectum (Rs portion) was closed with sutures and a loop sigmoid colostomy was created. We succeeded in saving the patient. He was discharged after 20 days.
