Abstract
OBJECTIVES: Decompressive craniectomy (DC) is an emergency life-saving procedure used to treat refractory intracranial hypertension (RICH). The authors aim to analyze their experience with protocol-based early DC (<24 h) in RICH cases diagnosed based on clinical and radiological evidence, without preoperative intracranial pressure monitoring done over 10 years.Entities:

Clinical criterion rule for raised intracranial pressure as per Alali et al. (2018)
A) Major criteria
Compressed cisterns (Marshall III)
Midline shift >5 mm (Marshall IV)
Nonevacuated mass lesion (>25 cm3)
B) Minor criteria
GCS motor score ≤4
Pupillary asymmetry
Abnormal pupillary reactivity
Marshall II (i.e., basal cisterns are present with midline shift of 0-5 mm and/or high- or mixed-density lesion of ≥25 cm3)
ICP – Intracranial pressure; GCS – Glasgow Coma Scale
